The findings include:

Seven client records were reviewed on July 28-29, 2014. All seven client records were required to include an informed and voluntary consent from the client prior to the disclosure of information. A consent to release information was not obtained in record #3.

Client #3 was admitted to treatment on 1-27-2014, and was discharged on 7-18-2014. A release of information for a funding source was not obtained prior to disclosure.

The findings were reviewed with the director and were not disputed.

Plan of Correction
The Program director contacted the identified client and disclosed to he/she that confidential information was exchanged with her funding source for payment while in treatment. The Program Director in staffing on 8-5-14 also retrained all staff in confidentiality and the process of obtaining the needed the documentation before a disclosure can occur. The client did come into office and sign a confidentiality statement for future contacts with this particular funding source if needed. Chart reviews for current documentation will occur on a monthly basis to prevent missed paperwork and other documentation.
