INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on April 22, 2026, by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection N8ZP6701 A New Tomorrow was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this inspection, and a plan of correction is required.

Plan of Correction

§ 717.29(e)(3)  LICENSURE Physical plant standards - Bathrooms

§ 717.29. Physical plant standards. (e) Bathrooms. The licensee shall: (3) Have hot and cold water under pressure. Hot water temperature may not exceed 120°F.
Observations
Based on a physical plant inspection, the facility failed to keep the water temperature from exceeding 120 degrees Fahrenheit. The bathroom had a water temperature of 138 degrees Fahrenheit. These findings were reviewed with the recovery house staff during the licensing process. This is a repeat citation from May 6, 2025, licensing inspection.

Plan of Correction
The hot water heater has been inspected by a new contractor to turn down to lowest setting. This was confirmed by a contractor hired to complete the task from last year's inspection to turn it down and checked monthly afterwards. It was reported to us it was turned down. This hot water heater does not have actual degree settings but instead 3 notches for high, medium, low. It is set now at the lowest. The house manager will continue to monitor the setting monthly by the purchase a thermometer to test the water temperature during the monthly inspections.

§ 717.30(b)(1)  LICENSURE Safety and emergency procedures - Exits

§ 717.30. Safety and emergency procedures. (b) Exits. The licensee shall: (1) Ensure that stairways, hallways and exits from rooms and from the drug and alcohol recovery house are unobstructed.
Observations
Based on a physical plant inspection, the facility failed to ensure that stairways, hallways and exits from rooms and from the drug and alcohol recovery house are unobstructed. The emergency exit on the second and third floor was obstructed and not easily accessible. These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
The 2nd floor window that had an AC blocking the access to the fire exit was cleared of obstruction by removing the AC in the window. There is not a need for the AC in that room as there is central air inside the house.

The 3rd floor window fire exit had a pathway obstruction of end stand dressers. The endstands were situated a different way to ensure a clear path to the fire exit window. During monthly inspections by the house manager, all exits, stairways, and hallways will be checked to ensure pathways are not obstructed

§ 717.30(b)(3)  LICENSURE Safety and emergency procedures - Exits

§ 717.30. Safety and emergency procedures. (b) Exits. The licensee shall: (3) Maintain each ramp, interior stairway and outside steps exceeding two steps with a well-secured handrail and maintain each porch that has over an 18-inch drop with a well-secured railing.
Observations
Based on a physical plant inspection, the facility failed to maintain each ramp, interior stairway and outside steps exceeding two steps with a well-secured handrail and maintain each porch that has over an 18-inch drop with a well-secured railing. The railing from the 2nd floor to the 3rd floor was loose and not secured. These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
On 4/28/2026, the railing from 2nd going to 3rd floor had a loose screw at the top. The railing screw was tightened and now the railing is secured. Going forward, during monthly house inspections, the house manager will check the railing to ensure it is still tight and secured.
