Observations
Based on the review of client records, the facility failed to document an informed and voluntary consent to release information form prior to the disclosure of information in one out of seven records reviewed. The client record did not contain a release of information form for the funding source. Facility staff confirmed billing had occurred. Client #5 was admitted on November 22, 2023 and was still active at the time of the inspection.The finding was reviewed with facility staff during the licensing inspection.This is a repeat citation from the August 24, 2023 licensing inspection.

Plan of Correction
Executive Director reviewed finding with Clinical Supervisor. Clinical Supervisor had clinical staff meet with client #5 to sign release of information for funding source on 1/31/24. Clinical Director will supervise clinical staff to make sure all releases for funding sources are signed by clients upon admission. The facility will be in full compliance by 1/31/24. |
