This finding was reviewed with facility staff during the licensing process.

Plan of Correction
Facility Director and Clinical Supervisor issued a policy memo on 12.13.23 to Restore Clinical Staff which reviews policy 709.63 requirements. Memo was reviewed by all clinical team members and acknowledged by signature and date.

Memo was also reviewed at Clinical Team meeting on (12/18/23).

Clinical Supervisor is responsible for ensuring plan is followed and will complete weekly audit of charts for those clients discharged in the past 7 days.

Clinical staff failure to comply with policy would result in disciplinary action.
