This report is a result of an renewal on-site licensure inspection conducted on October 7, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, HGSG6701 Unity Sober Living Homes was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.

§ 717.20. Resident records.
(a) The licensee shall maintain an individual record for each resident which must include:
(1) All records obtained under § 717.22.

The recovery house failed to maintain an individual record for each resident.

Plan of Correction
The Facility acknowledges the deficency in maintaining an effective filing system we recognize the need for improvement and are actively implementing a more ogranized process which will be completed by 11/1/2025. To ensure that all intake packets and related paperwork are properly managed and safeguarded. Moving forward we are committed to maintaining accurate records long term and preventing the loss or misplacement of any documentation. |
