Observations
Based on a review of personnel records, the facility failed to document HIV/AIDS and TB/STD training in one of two personnel records.

The findings include:

Two staff records were reviewed on March 4, 2015. Both staff records required documentation of HIV/AIDS and TB/STD training. Staff persons and volunteers shall receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum. Counselors and counselor assistants shall complete the training within the first year of employment. The facility failed to document HIV/AIDS and TB/STD training in personnel record # 2.

Employee # 2 is a counselor and was hired on December 2, 2013. A Department approved curriculum in HIV/AIDS and TB/STD training was due to be completed no later than December 2, 2014. There was no documentation of the completion of HIV/AIDS and TB/STD training in the record of employee #2.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Operations Manager confirms that Employee # 2 did not have the required HIV/AIDS and TB/STD training within the first year of employment. The requirement for both trainings within the first year has been reviewed with all applicable staff, in their clinical supervision and staff meetings. All current therapeutic staff will have the trainings by 5/21/15. The Operations Manager and Business Manager will ensure that any future therapeutic staff will have both required trainings within the first year. The Business Manager will present both trainings in-house to all staff annually. This will be overseen by the Project Director.
