INITIAL COMMENTS
 
This report is a result of an on-site physical plant inspection conducted for a capacity increase request. The inspection was conducted on January 9, 2014 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment, Program Licensure Division. The facility will be notified by a separate letter of the determination regarding the capacity increase request.
