INITIAL COMMENTS
 
A follow-up survey conducted on June 9, 2009 found Philadelphia Teen Challenge Training Center, Inc. had corrected the deficiencies cited under chapters of 28 PA Code which pertain to the facility. The deficiencies were cited as a result of an on-site licensure renewal inspection conducted on February 06, 2009.
