|
704.12(a)(6) LICENSURE OutPatient Caseload
704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios.
(a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client.
(6) Outpatients. FTE counselor caseload for counseling in outpatient programs may not exceed 35 active clients.
|
Observations Based on a review of personnel records and the Staffing Requirements Facility Summary Report, the facility failed to ensure the full-time equivalent (FTE) counselor caseload for counseling in outpatient programs did not exceed 35 active clients in three of five applicable personnel records reviewed.
The counselor FTE is determined by dividing the total number of hours the counselor devotes to their clients by the facility's workweek. Then, in order to obtain the counselor's ratio, the total number of clients on the counselor's outpatient caseload is divided by the FTE.
Employee # 3 was hired as a clinical supervisor on March 4, 2024 and was still employed at the time of the inspection. Per the numbers reported in the Staffing Report, the employee's ratio calculation was as follows: 15/40 = 0.375(FTE); 30/0.375 = 80, which equals a FTE client/counselor ratio of 80:1.
Employee # 6 was hired as a counselor on July 1, 2025 and was still employed at the time of the inspection. Per the numbers reported in the Staffing Report, the employee's ratio calculation was as follows: 8/40 = 0.2(FTE); 15/0.2 = 75, which equals a FTE client/counselor ratio of 75:1.
Employee # 7 was hired as a counselor on July 1, 2025 and was still employed at the time of the inspection. Per the numbers reported in the Staffing Report, the employee's ratio calculation was as follows: 8/40 = 0.2(FTE); 15/0.2 = 75, which equals a FTE client/counselor ratio of 75:1.

Plan of Correction Clinic staff do understand the regulatory requirement and have designed a master client document tracking tool that is regularly updated and available to front desk staff and providers to mitigate fall outs. The challenge surrounding client appointment compliance continues to impact late treatment plan fallouts. Effectively immediately, the monitoring tool will be amended to reflect a 45 day treatment plan update (target) date to allow the clinic team additional time to improve compliance. Monthly, the Program and Facility Directors will review treatment plan compliance, based on 100% of active chart audits with the Episcopal Campus Executive Director. This audit cadence will continue until 4 consecutive months of 100% compliance is achieved. |
