Observations
Based on a review of personnel records, the facility failed to document TB/STD training for the project director in one of one personnel records.
Staff #1 was hired as the project director on July 1, 2019. TB/STD training was due to be completed by July 1, 2021. Staff #1 does not have documentation of TB/STD training.
These findings were reviewed with facility staff during the licensing process.
This is a repeat deficiency from the October 25, 2021 annual licensing inspection.

Plan of Correction
On 11/15/2022, Project director is registered for the TB/STD/Hep DDAP training. Upon completion of the training, the project director's training certification will be placed in her personnel records.
