The specific information to be released on consents to release information forms exceeded that allowable under 4 Pa. Code 255.5 in five of eight client records reviewed, #2, 3, 4, 5 and 6.

Plan of Correction
As of 12/8/2006, the SIL's plan of correction for the noted deficiency is in effect. We implemented a new consent form to release confidential information to the managed care companies only releasing the following information in accordance with 4 Pa. Code 255.5.
- The client is or is not in treatment.
- Prognosis of the client.
- The nature of the project.
- A brief discussion of the progress of the client.
- A short statement as to whether the client has relapsed into drug or alcohol abuse and the frequency of such relapse.
All completed consent forms regarding managed care companies will be supervised and approved by Clincal SIL staff 2 and 3 to insure that the information does not exceed the 4 PA code 255.5. |
