This report is a result of an complaint investigation conducted on December 2, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation which included a telephone interview with facility staff, Tadiso Inc. was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
