INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on December 17-20, 2018, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Caron Treatment Centers - Detox Unit was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

Plan of Correction

705.2 (2)  LICENSURE Building exterior and grounds.

705.2. Building exterior and grounds. The residential facility shall: (2) Keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well-being of residents, employees and visitors. The exterior of the building and the building grounds or yard shall be free of hazards.
Observations
During a physical plant inspection conducted on December 20, 2018 within the Carole Neag building, an outdoor egress area was observed to be of hazard for residents, employees, and visitors.

The exit egress consisted of an uneven earthen path that was sloping and narrow in some locations which would present a hazard when exiting especially for wheel chair dependant individuals, particularly during inclement weather conditions.

These findings were reviewed with facility staff during the licensure inspection.

Plan of Correction
The uneven earthen path from the Neag exit will be leveled and a path of 2A stone and P gravel will be placed until a paved asphalt path can be installed. This stone and gravel path will be completed by March 1, 2019.

Weather permitting, a paved path will be installed by April 1, 2019 from the Neag building exit egress to the paved driveway. This paved path will allow individuals who are wheel chair dependent to exit the building in emergency situations. The Corporate Director of Facilities will ensure all work is completed per the plan of correction.
