Observations An interview was conducted with facility staff on November 15, 2017. Based on this review, the facility failed to obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record.
Facility staff reported that its physician released urinalysis results to another health care provider prior to attaining the client's permission to make the disclosure.

Plan of Correction While the physician did have a signed consent form from the patient, it did not include the urinalysis results as an item that could be discussed with the other provider. Our physician was provided with additional training regarding applicable confidentiality regulations as it pertains to the release of information from the patient record. This additional training took place on 11/15/17. He is now aware that the written release form must identify each specific piece of information to be released/discussed. The Program Director will monitor the physician's adherence to applicable confidentiality regulations. |
