This report is a result of an on-site licensure renewal inspection conducted on April 4, 2022 by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Human Services Administration Organization Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

705.28 (c) (3) LICENSURE Fire safety.
705.28. Fire safety.
(c) Fire extinguishers. The nonresidential facility shall:
(3) Ensure fire extinguishers are inspected and approved annually by the local fire department or fire extinguisher company. The date of the inspection shall be indicated on the extinguisher or inspection tag. If a fire extinguisher is found to be inoperable, it shall be replaced or repaired within 48 hours of the time it was found to be inoperable.

Observations
Based on a physical plant inspection conducted with the facility director on April 4, 2022 at approximately 9:30 AM, it was observed the facility ' s fire extinguishers were not inspected annually by the local fire department or fire extinguisher company.
The facility has four fire extinguishers that were last inspected in January 2021. The fire extinguishers were due to be inspected no later than January 31, 2022.
The findings were reviewed with facility staff during the licensing inspection.

Plan of Correction
Fire Extinguisher inspections will be conducted annually unless otherwise indicated by DDAP.
Compliance with this expectation will be monitored by the Project Director and/or the Facility Director to ensure that Property Management is scheduling said inspections.
Facility is in full compliance with this expectation as of 4/8/2022. |
