INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection and methadone monitoring inspection conducted on February 4 - 6, 2019 by staff from the Department of Drug and Alcohol Programs, Bureau of Quality Assurance for Prevention and Treatment. Based on the findings of the on-site inspection, Fayette Treatment Center LLC d/b/a Greene Treatment Center was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection.

Plan of Correction

715.14(a)  LICENSURE Urine testing

(a) A narcotic treatment program shall complete an initial drug-screening urinalysis for each prospective patient and a random urinalysis at least monthly thereafter.
Observations
The facility failed to document a random drug screening urinalysis at least monthly in one of twelve patient records reviewed on February 4 - 6, 2019.

Patient # 9 was admitted October 17, 2018. No drug screens were documented for November 2018, December 2018 and January 2019.

A nursing note from November 2018 indicted the patient refused to complete a drug screen.

Nursing notes from December 2018 and January 2019 stated the physician indicated the patient had an exception and could not complete the drug screens due to medical reasons.

The facility had not been granted an exception by the Department as of February 6, 2019.

The findings were reviewed with facility staff during the licensing process.

Plan of Correction
The Greene Treatment Center failed to document a random drug screening urinalysis at least monthly in one of twelve patient records reviewed on February 4 - 6, 2019.

Patient #9 was admitted October 17, 2018. No drug screens were documented for November 2018, December 2018, and January 2019. The Greene Treatment Center had not been granted an exception by the Department as of February 6, 2019.

A general exception request was submitted to DDAP - as per regulation 715.14 (a) - requesting to use oral swabs instead of urinalysis for patients with medical conditions which prevent them from completing random urinalysis at least monthly. The general exception request was approved and signed by the Greene Treatment Center Board of Directors.

The Greene Treatment Center Project Director then submitted the general exception request to DDAP via email. Greene Treatment Center is currently awaiting DDAP exception approval. Since the exception has not yet been granted, the Lead LPN will ensure that all patients will complete a random monthly urinalysis thereafter, unless the exception is approved.

Patients with medical conditions that fall under the exception will receive a monthly oral swab testing. The CRNP will ensure that the required verification of the medical condition will be documented in the patient's electronic medical record.
