INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on November 17, 2022, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Anthony Pater Counseling was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

(ii) Confidentiality. (iii) Pharmacology. (iv) Treatment planning. (v) Counseling techniques. (vi) Drug and alcohol assessment. (vii) Codependency. (viii) Adult Children of Alcoholics (ACOA) issues. (3) If a counselor has been designated as lead counselor supervising other counselors, the training shall include courses appropriate to the functions of this position and a Department approved core curriculum or comparable training in supervision.
Observations
Based on a review of one personnel record, the sole proprietor failed to complete 25 clock hours of training annually.

Employee #1 was hired as the Project Director/ Facility Director/ Counselor on February 1, 2012 and is current in that position. Employee #1 is the sole proprietor and responsible for completing 25 clock hours of training annually. During 2021, only 10 clock hours were documented.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
In order to assure that the owner/project director/counselor will achieve the 25 hours of required education each year, a reminder will be placed in the counselor's monthly objective list to check for and sign up for appropriate trainings. This change will become effective December 1, 2022.

705.28 (d) (4)  LICENSURE Fire safety.

705.28. Fire safety. (d) Fire drills. The nonresidential facility shall: (4) Maintain a written fire drill record including the date, time, the amount of time it took for evacuation, the exit route used, the number of persons in the facility at the time of the drill, problems encountered and whether the fire alarm or smoke detector was operative.
Observations
Based on a review of the written fire drill record from December 2021 to October 2022, the facility failed to include the time the fire drill occurred for each unannounced fire drill recorded.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The fire drill log has been changed by the owner/counselor/project director so that "time of day" has its own column and therefore will not be missed. This form will become effective as of 11/18/22.
