This report is a result of an on-site recovery house complaint investigation conducted on September 11, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, TNH 7G0W6701 was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house.

§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(3) Keep the drug and alcohol recovery house free of rodent and insect infestation.

Observations
Based on an inspection of the physical plant is was determined the recovery house failed to keep the drug and alcohol recovery house free of insect infestation as cockroach carcasses were observed in the kitchen area.

Plan of Correction
Pest control frequency has been increased to bi-monthly with additional visits to be scheduled when/if increased activity is observed. Management visits have been increased to twice a week to ensure food and trash are properly being taken care of. Attached home has pest problems, the pests are finding their way through the wall. |
