This report is a result of an on-site follow-up investigation conducted on September 23, 2016 by staff from the Division of Accountability and Program Improvement in response to Incident Reports that were submitted by the facility. Based on the findings of the investigation, Gaudenzia, Inc-Sunbury responded appropriately to the incident. The facility was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
