Based on a Relicensure Survey completed on August 29, 2023, it was determined that Reading Midwife Center for Women's Wellness, Llc, was not in compliance with the following requirements of the Life Safety Code for an existing birth center health care occupancy.

This is a two-story, Type V (111), protected wood frame structure, with a basement, which is not sprinklered.

 STANDARD
Building Construction

Minimum construction standards (NFPA 220), Standard types of building construction:
(1) Buildings of one story in height only may be constructed of fire resistive, non-combustible, ordinary or wood frame construction.

(2) Buildings of two (2) stories or more in height shall be constructed of fire resistive or noncombustible construction


Observations:

Based on observation and interview, it was determined the facility failed to meet the required construction type, in accordance with NFPA 101 and NFPA 220 for the component.

Findings include:

1. Observation on August 29, 2023, at 11:00 AM, revealed the facility exceeded the maximum allowable story height for this type of building construction.

Exception #93-0452 previously assigned.

Interview with the Administrator on August 29, 2023, at 11:00 AM, confirmed the building height exceeded the allowable height.




Plan of Correction:

We request Exception be accepted #93-0452 as previously assigned.


 STANDARD
Means of Egress

Means of Egress Requirements

(1) Each floor or fire section of the building shall have at least two approved means of egress, remotely located from each other.
A- At least on means of egress shall be a stairway directly to grade, door directly to grade, horizontal exit or ramp directly to grade.
B- The other means of egress shall be an approved fire escape or outside stair if two means of egress are not provided in accordance with section 501.40 (B) (1) (a).

(2) Width of stairways, fire escapes and ramps:
A- Stairs shall have a minimum width of 30 inches. Winders in stairs are not acceptable.
B- Fire escapes shall have a minimum width of 24 inches. Fire escapes shall be constructed of steel.
C- Ramps shall have a minimum width of 30 inches. Slope of ramp shall not exceed 2 inches in 12 inches.
D- Handrails shall be provided for 501.40 (B) (2) (a) (b) and (c). Handrails may extend into the stairs not more than 3 inches on either side.

Observations:

Based on observation and interview, it was determined the facility failed to provide two means of egress, on two of three floors within the component.

Findings include:

1. Observation on August 29, 2023, at 11:15 AM, revealed the basement lacked two acceptable means of egress.

Exception #93-0453 previously assigned.

Interview with the Executive Director on August 29, 2023, at 11:15 AM, confirmed the lack of two acceptable means of egress, from the basement.
