Initial Comments:


Based on the findings of an onsite unannounced state relicensure survey completed 11/11/2022, Anew Home Health Agency Inc. was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart G, Chapter 601, Home Health Care Agencies.

Based on the findings of an on-site unannounced State relicensure survey completed 11/11/2022, Anew Home Health Agency Inc. was found not to be in compliance with the following requirement of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

51.3 Notification

(f) If a health care facility is
aware of a situation or the occurrence
of an event at the facility which
could seriously compromise quality
assurance or patient safety, the
facility shall immediately notify the
Department in writing.
The notification shall include
sufficient detail and information to
alert the Department as to the reason
for its occurrence and the steps which
the health care facility shall take to
rectify the situation.

Observations:


Based on review of Pennsylvania (Pa.) Department of Health (DOH) Event Reporting System (ERS), agency documents and staff (EMP) interview the agency failed to ensure event reporting to the Pennsylvania (Pa.) Department of Health ERS system for nineteen (19) staff and forty eight (48) patients who were diagnosed COVID-19 positive from November 2020 to 11/8/2022.

Findings Included:

Review of the departments event reporting system guide on 11/10/2022 at approximately 3:30 PM revealed, "Health department Reportable Diseases COVID-19 Quick Start Guide COVID-19 records are reportable within 24 hours after being diagnosed ...Health Department Reportable Diseases (Covid-19) - If you select Event Type = Health Department Reportable Diseases and Event-Sub-type = Covid-19, the Reportable Disease Detail section will appear. You will be required to add at least one Reportable Disease Detail record before submitting the event to DOH. There are two ways to enter information. It should be noted that both methods can be used during the same event. You can manually enter data and/or upload a spreadsheet/s..."

During an interview with EMP1 and EMP2 on 11/9/2022 at approximately 9:35 AM EMP1 and EMP2 confirmed no Covid-19 positive staff or patients were reported through the ERS system. The surveyor asked for any documentation pertaining to reporting of Covid-19 positive cases for patients and staff. A document listed "Covid Positive Patients..." was provided by EMP2 listing staff and patients from June 2020 to current which confirmed the following: nineteen (19) staff and forty eight (48) patients who were diagnosed COVID-19 positive from November 2020 to 11/8/2022.

An exit interview was conducted with the administrator, human resources manager and marketing/IT manager on 11/10/2022 at approximately 1:30 PM which confirmed the above findings.








Plan of Correction:

Corrective action began on 11/09/2022, immediately following discussion with and confirmation from the DOH Representative.
Staff or patients who have a new diagnosis of Covid-19 are tracked by the Quality Assurance Manager.
The information gathered consists of the individual's name and date of the positive Covid-19 diagnosis.
This information is added to an electronic spreadsheet and to a physical Covid-19 tracking binder which are kept within the Quality Assurance Manager's office.
Following review of the new information, the Administrator will report the information, within 24 hours, to the PA Department of Health, via the ERS system either manually or spreadsheet format, depending on the number of individuals being reported at that time.
This process will be incorporated into our Quality Assessment Performance Improvement committee meetings to be reviewed, discussed, and re-evaluated on a quarterly basis.
This process will continue daily throughout the public health emergency or as required.
