Initial Comments:
An off-site, state licensure follow-up survey conducted on May 13, 15 and 20, 2024 found that Associated Family Home Care, Inc. had corrected the deficiencies cited under 28 PA Code, Part IV, Health Facilities, Subpart G. Chapter 601. The deficiencies were cited as a result of a state re-licensure survey completed on March 14, 2024.
