Based on an Initial Licensure completed on November 7, 2023, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for a new Ambulatory health care occupancy.
This Final Occupancy Survey was conducted for Documentation to reopen a 4,700 square foot Class B ASF under new management. Facility closed on 03/31/2023.

This is a one-story, Type II (000), unprotected non-combustible construction, which is non-sprinklered.
Approved as a Class B Ambulatory Surgical Facility.
