Initial Comments:

This report is the result of an occupancy survey conducted on August 1, 2024, at Cardiovascular Experts, which included adding the following procedures:
CPT 33207/0525T/0526T/0527T: Insertion of new or replacement of permanent pacemaker or intracardiac monitoring system with transvenous electrode(s); ventricular; CPT 33208: Insertion of new or replacement of permanent pacemaker with transvenous electrode(s); CPTs 33212/33213/33221: Insertion of pulse generator, single leads/dual leads/multiple leads; CPT 33215: Reposition previously implanted transvenous right atrial or right ventricular lead, permanent pacemaker or implantable defibrillator; CPTs 33216/33217/33249/33270/33271: Insertion of a single/dual transvenous electrode(s), permanent pacemaker or implantable defibrillator; CPTs 33218/33220/33273: Repair/Reposition of single/dual transvenous electrode(s), permanent pacemaker or implantable defibrillator; CPTs 33224: Insert LV lead, attach to previously placed generator; CPTs 33227/33228/33229/0530T/0531T/0532T: Removal of permanent pacemaker pulse generator with replacement of pacemaker pulse generator; single lead system (33227) dual lead system (33228) multiple lead system (33229); Removal of intracardiac ischemic monitoring system-electrodes, leads, monitor (0530T/0531T/0532T); CPTs 33230/33231/33240: Insertion of implantable defibrillator pulse generator only; with existing dual leads (33230), with existing multiple leads (33231), single leads (33240); CPT 33289: Implantation of Hemodynamic Monitor.
Based on the occupancy survey, it was determined the facility was in compliance with all applicable requirements of the Pennsylvania Department of Health's Rules and Regulations for Ambulatory Care Facilities, Annex A, Title 28, Part IV, Subparts A and F, Chapters 551-573, November 1999 and the current edition of the Guidelines for Design and Construction of Outpatient Facilities.
