Initial Comments:


This report is the result of an onsite occupancy survey conducted on May 22, 2025, and concluded offsite on May 27, 2025, at outpatient health center located at 60 Cherry St. Leesport, PA, 19533 which included CareNow, Outpatient Imaging- Xray, Outpatient Women ' s Imaging- Mammography and Dexascan, Outpatient Laboratory, and Physical Therapy. Based on the occupancy survey, it was determined the facility was in compliance with all applicable requirements of the Pennsylvania Department of Health's Rules and Regulations for Hospitals, 28 Pa Code, Part IV, Subparts A and B, November 1987, as amended June 1998 and the current edition of the Guidelines for Design and Construction of Hospital and Health Care Facilities.

This report is for new equipment, automated external defibrillator (AED) x2, benchtop centrifuge, dexascan system, mammography system, xray system, occupational sounds test enclosure, audiometer, air spirometry system, hand-held breath tester, electrocardiogram (EKG) machine, blood glucose monitor, vital signs system, and medication dispensing cabinet, beginning on May 28, 2025. The Saint Luke ' s Hospital of Bethlehem, Pennsylvania attested they were in full compliance with the requirements of the Pennsylvania Department of Health's Rules and Regulations for Hospitals, 28 PA Code, Part IV, Subparts A and B, November 1987, as amended June 1998.
