Based on the findings of an onsite state re-licensure survey conducted on April 24, 2025, Care R Us, LLC, was found not to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.
Plan of Correction:
611.56(a) LICENSURE Health Screening Name - Component - 00 (a) A home care agency or home care registry shall insure that each direct care worker and other office staff or contractors with direct consumer contact, prior to consumer contact, provide documentation that the individual has been screened for and is free from active mycobacterium tuberculosis.
Observations:
Based upon review of direct care worker personnel files (PF) and an interview with the agency owners, the agency failed to ensure that a second step TB baseline (preplacement) screening was conducted for two (2) of seven (7) PF reviewed. (PF# 3 and 4).
Findings Include:
The CDC (Centers for Disease and Control and Prevention) guidelines state baseline (preplacement) screening and testing, in addition to the IGRA (interferon-gamma release assay) or tuberculin skin test (TST), shall include a symptom screen questionnaire and an individual tuberculosis (TB) risk assessment. Serial screening and testing not routinely recommended. Annual TB education is recommended. (CDC/MMWR/May 17, 2019/Vol. 68/No. 19).
A review of PF's conducted on April 25, 2025, from approximately 12:00pm - 12:30 pm revealed the following:
PF #3, date of hire 03/04/2025, contained documentation of only one baseline (pre-placement) TB testing dated 02/23/2025, and no documentation of a second step TB test.
PF #4, date of hire 10/27/2023 contained documentation of only one baseline (pre-placement) TB testing dated 03/13/2023, and no documentation of a second step TB test.
An interview with the agency administrator on April 24, 2025, at approximately 12:45 pm confirmed the above findings.
Plan of Correction:On April 24th we contacted 2 direct care workers that was missing a TB 2-step and directed them to the nearest urgent care facility to have a quantiferon tb gold blood test taken immediately. We explained to both direct care workers that it is a mandated requirement as per CDC guidelines. We also provided TB education via email. We've have taken both caregivers off the schedule until the results are submitted. We will immediately require all future direct care workers to submit a quantiferon gold TB blood test upon hire instead of a TB 2-step to prevent any overlooks. In addition to making the TB blood test a requirement, we will also give financial assistance to any direct care worker who finds it difficult to pay for the test. Human Resources will use a Personnel Audit tool (A complete list of forms that is needed to complete personnel file) to conduct an independent audit 7 days after a date of hire of a direct care worker and every 90 days for a maximum of six months to ensure that every employee has a TB blood test in their employee file.
