Initial Comments:


Based on the findings of an onsite unannounced State relicensure survey completed 5/25/2022, Cove Home Care Llc. was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an onsite unannounced state relicensure survey completed 5/25/2022, Cove Home Care Llc. was found not to be in compliance with the following requirement of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries.

611.51(b) LICENSURE
Direct Care Worker Files

Name - Component - 00
Files for direct care workers employed or rostered shall include documentation of the date of the face-to-face interview with the individual and of references obtained. Direct Care Worker files also shall include other information as required by 611.52, 611.53, if applicable, 611.54, 611.55 and 611.56 (relating to criminal background checks, child abuse clearance, provisional hiring, competency requirements; and health evaluations).

Observations:


Based on review of policy, personnel files (PF) and staff (EMP interview, it was determined the agency failed to include documentation of the date references were obtained for three (3) of five (5) reviewed (PF1, PF2 and PF3).

Findings included:

Review of the agency policy on 5/23/2022 at approximately 11:30 AM revealed,
"EMPLOYEE HANDBOOK AND COMPANY GUIDELINES...JOB DESCRIPTION...QUALIFICATIONS: Minimum of ninth education; high school diploma or GED preferred. Must demonstrate satisfactory completions of any stated mandated training. Application must be bondable and meet or exceed minimum qualifications for each of the following background checks; Criminal Background Investigation, Motor Vehicle Driving Record, Credit History, Professional and Personal Reference Checks..."

A personnel file (PF) review was conducted on 5/23/2022 from approximately 11:30 AM to 12:30 PM.

PF#1 date of hire (DOH) 4/26/2020, two references were documented, but the reference dates could not be confirmed.

PF#2 DOH 10/1/2020, two references were documented, but the reference dates could not be confirmed.

PF#3 DOH 4/26/2021, two references were documented, but the reference dates could not be confirmed.

An interview was conducted with the administrator and office manager on 5/23/2022 at approximately 2:05 PM which confirmed the above findings.











Plan of Correction:

1. The agency will update the current Reference Check Form to add a section for the employer completing form to add their name and date of completion to assure date will not be missed on any new references being verified within the hiring process at said agency. (July 6, 2022).
2. The agency will review all current and past employee charts for any unmarked dates on said Reference Check Forms and tag them in their chart for future surveys. Complete within 30 days (July 6, 2022).
3. Owner/Administrator will be made responsible to monitor the continued implementation of the plan of correction.
4. Within 30 days (July 6, 2022) all corrections and plans will be in place.


Initial Comments:


Based on the findings of an onsite unannounced State relicensure survey completed 5/25/2022, Cove Home Care Llc was found to be in compliance with the requirements of 35 P.S. 448.809 (b).
