Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.
Plan of Correction:
611.56(b) LICENSURE Health Screening Name - Component - 00 (b) A home care agency or home care registry shall require each direct care worker, and other office staff or contractors with direct consumer contact, to update the documentation required under subsection (a) at least every 12 months and provide the documentation to the agency or registry. The 12 months must run from the date of the last evaluation. The documentation required under subsection (a) shall be included in the individual's file.
Observations:
Based on a review of agency personnel files (PF), CDC (Centers for Disease Control) guidance and staff (EMP) interview, it was determined the agency failed to ensure Tuberculosis (TB) education provided annually for two (2) of two (2) PF files reviewed with at least 12 months of employment (PF4 & 5).
Findings included:
The CDC guidelines state that baseline (preplacement) screening and testing, in addition to the IGRA (interferon-gamma release assay) or TST, shall include a symptom screen questionnaire and an individual TB risk assessment. Serial screening and testing not routinely recommended. Annual TB education is recommended. (CDC/MMWR/May 17, 2019/Vol. 68/No. 19).
Review of PF completed on November 22, 2024, at approximately 10:30 am revealed:
PF4, date of hire (DOH) 6/30/21, PF failed to contain evidence of annual TB education for 2022, 2023, and 2024.
PF5, DOH 10/30/23, PF failed to contain evidence of annual TB education for 2024.
Exit interview with owner on November 22, 2024, at approximately 3:00 pm confirmed findings.
Plan of Correction:The owner will ensure that all staff are fully informed about tuberculosis (TB), its transmission, and the significance of annual screening to enable effective communication about TB. The owner will: 1. Assessment and Evaluation - Review the TB screenings from the past year, including the number of screenings, identified positive cases, and any required follow-up actions. 2. Screening - Assess the current screening process for thoroughness and adherence to the latest health guidelines. 3. Training Needs - Determine any gaps in staff training regarding TB awareness, screening methods, and management of positive cases. 4. Training - Conduct comprehensive TB management training sessions for all healthcare workers, with regular, hands-on training to ensure staff awareness. 5. Audits - Perform monthly audits of TB screenings to verify that all staff have completed their annual education by the anniversary date of their TB test. Provide a written or multiple-choice test in line with current CDC Guidelines, which all staff must acknowledge by signing off. 6. Annual Review - Carry out a yearly evaluation of the TB health screening program to assess its effectiveness and implement necessary adjustments.
