Initial Comments:

Based on the findings of an onsite, state re-licensure survey conducted on May 24, 2024, Carebuilders at Home was found to be in compliance with the requirements of 28 PA Code, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an onsite, state re-licensure survey conducted on May 24, 2024, Carebuilders at Home was found not to be in compliance with the following requirement of 28 PA Code, Part IV, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries.

611.56(a) LICENSURE
Health Screening

Name - Component - 00
The screening shall be conducted in accordance with CDC guidelines for preventing the transmission of mycobacterium tuberculosis in health care settings. The documentation must indicate the date of the screening which may not be more than 1 year prior to the individual's start date.

Observations:

Based on review of the agency policy and procedure manual, consumer (agency) and personnel files, tuberculosis (TB) screening guidance from the Centers for Disease Control (CDC) and based on interview with the administrator (Employee #8), the agency failed to ensure initial TB screening was performed in accordance with CDC guidelines for four (4) of four (4) direct care workers (personal care aide) who were assigned to provide home care services after June 17, 2021. (Employees #1, #3, #6 and #7)

Finding include:

On May 24, 2024 at approximately 1:55 PM, review of the CDC webpage ( https://www.cdc.gov/tb-healthcare-settings/hcp/screening-testing/baseline-testing.html?CDC_AAref_Val=https://www.cdc.gov/tb/topic/infectioncontrol/healthcarepersonnel-faq.htm )revealed initial TB screening recommendations included, but were not limited to, the following:
-Health Care Personnel Baseline Individual TB Risk Assessment; and
-TB Symptom Evaluation.

On May 24, 2024 at approximately 1:28 PM, review of agency policy 501, titled "Tuberculosis Prevention, Control Plan and Risk Assessment Pennsylvania 611.56 CDC Guidance on Prevention and Control of Tuberculosis":
I. Tuberculosis Screening and Control Policy...B. The screening shall be conducted in accordance with CDC guidelines.

Consumer #1: On May 24, 2024 at approximately 9:52 AM, review of the consumer file revealed the DCW (Employee #1) provided home care services in April and May 2024.

Consumer #2: On May 24, 2024 at approximately 10:17 AM, review of the consumer file revealed the DCW (Employees #3) provided home care services in April and May 2024.

Consumer #4: On May 24, 2024 at approximately 10:47 AM, review of the consumer file revealed the DCW (Employees #6) provided home care services in April and May 2024.

Consumer #6: On May 24, 2024 at approximately 11:15 AM, review of the consumer file revealed the DCW (Employee #7) provided home care services in May 2024.

Review of personnel files on May 24, 2024 at approximately 12:05 PM revealed the following:
Employee #1: The first date of assignment of the DCW to provide home care services was 12/27/2021. There was no documentation in the personnel file which provided evidence a TB symptom screening had been completed prior to the assignment to provide home care services.
Employee #3: The first date of assignment of the DCW to provide home care services was 08/02/2023. There was no documentation in the personnel file which provided evidence a TB symptom screening had been completed prior to the assignment to provide home care services.
Employee #6: The first date of assignment of the DCW to provide home care services was 01/30/2023. There was no documentation in the personnel file which provided evidence a TB symptom screening had been completed prior to the assignment to provide home care services.
Employee #7: The first date of assignment of the DCW to provide home care services was 05/06/2024. There was no documentation in the personnel file which provided evidence an individual TB risk assessment had been completed prior to the assignment to provide home care services.

During interview on May 24, 2024 at approximately 1:30 PM, the administrator confirmed an individual TB risk assessment or the TB symptom screening were not completed prior to the assignment of the above referenced DCW's to provide home care services.







Plan of Correction:

Corrective Action: The Director has added the CDC TB Risk assessment and a TB Screening to each new hire orientation packet as of 5/24/2024. The TB risk assessment and screening form will be completed for any new employees prior to starting an assignment.

The Director will sign off on all employee files prior to the start of assignment to ensure all mandatory forms/documents are completed.
