Based on an Emergency Preparedness Survey completed on June 27, 2022, at BARC Developmental Services Inc. Old Bethlehem Ii, it was determined there were no deficiencies identified with the requirements of 42 CFR 483.475.

Based on a Medicaid Recertification Survey completed on June 27, 2022, it was determined BARC Developmental Services Inc. Old Bethlehem Ii, was not in compliance with the following requirements of the Life Safety Code for an existing ICF/IID health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.470(j).

This is a one-story, Type V (000), unprotected wood frame construction, with a basement, which is fully sprinklered.

State Plans approved as Impractical.

Equipment using gas or related gas piping complies with NFPA 54, National Fuel Gas Code, electrical wiring and equipment complies with NPFA 70, National Electric Code.

Observations:

Based on observation and interview, it was determined the facility failed to maintain electrical outlets in two locations.

Findings include:

1. Observation on June 27, 2022, 1:15 pm, revealed beds in rooms 2 and 3 were placed against electrical outlets.

Interview on June 27, 2022, at 1:15 pm, with the facility representative confirmed the beds were in contact with the electrical outlets.





Plan of Correction:

On 6-28-22, the outlets in rooms 2 and 3 were beds are placed were removed and replaced with solid plates with no access to the boxes behind them.

The monthly Home Inspection Checklist has been revised to include a review of each room to indicate if any type of bed is pushed up directly next to an outlet on any side of the bed. If this is discovered, a Work Order will be put in immediately and the Maintenance Manager is notified immediately. Additionaly, the Home Inspection Checklist is reviewed by the Maintenance Manager each month and any outlets needing to be removed will be removed as required.
