Based on an Emergency Preparedness survey completed on September 20, 2022, at BARC Developmental Services Inc. Zion Hill, it was determined there were no deficiencies identified with the requirements of 42 CFR 483.475.

Based on a Medicaid Recertification Survey completed on September 20, 2022, it was determined BARC- Zion Hill was not in compliance with the following requirements of the Life Safety Code for an existing ICF/IID health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 483.470(j).

This is a one-story, Type V (111), protected wood frame construction, with a basement, which is sprinklered, with the exception of closets and small bathrooms.

State plans approved as Prompt.

In Prompt Evacuation Capability facilities, there are no construction requirements.

Observations:

Based on observation and interview, it was determined the facility failed to maintain the building construction requirements, affecting one of two levels within the facility.

Findings Include:

1. Observation on September 20, 2022, at 10:00 a.m., revealed two holes, Basement, drywall ceiling. The holes had plastic caps covering the holes. The facility is classified as a Type V (III), protected wood frame construction, which requires a one hour fire resistance rating.

Exit Interview with the Facility Maintenance Manager on September 20, 2022, at 10:30 a.m., confirmed the unsealed penetrations of the rated ceiling assembly.







Plan of Correction:

-The holes in the ceiling were repaired with 5/8 inch fire code rated drywall u-sing D-3850 5/8 inch FSW resistant classification. This was completed on 9-22-22. All future repairs will be made with this type of drywall.

Designated means of escape shall be continuously maintained clear of obstructions and impediments to full instant use in the case of fire or emergency.

Observations:

Based on observation and interview, it was determined the facility failed to ensure exit doors did not require excessive force to open, affecting one of two levels within the component.

Findings Include:

1. Observation on September 20, 2022, at 10:15 a.m., revealed First Floor, hall ramp exit door required excessive force to open. Refer to NFPA 101, Section 7.2.4.5

Exit Interview with the Facility Maintenance Manager on September 20, 2022, at 10:30 a.m., confirmed the egress door required excessive force to open.






Plan of Correction:

The hall ramp exit door was adjusted by tightening all hardware. Upon inspection, it was found to open easily as required.

Checking all doors is now part of the monthly home checklist.
