Initial Comments:



Based on the findings of an onsite unannounced complaint investigation survey conducted on March 10, 2020 and offsite March 11, 2020, Connect A Care LLC, was found NOT to be in compliance with the following requirement of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

51.3 Notification

(a) A health care facility shall
notify the Department in writing at
least 60 days prior to the intended
commencement of a health care service
which has not been previously provided
at that facility.

Observations:


Based upon review of Department files, an on-site visit, and interviews, it was determined that the agency failed to notify the Department of a change in address in writing at least 60 days prior to the intended commencement.

Finding include:

On March 9, 2020, a Department file review of an undated change of address letter, revealed that, "On 9/30/18 the agency would be moving from 6607 Chew Avenue, Philadelphia Pa., to 1542 Haines Street Philadelphia Pa."

On March 9, 2020, a Department file review of a second undated change of address letter , revealed that, "On 10/01/2018 the agency would be moving from 7934-36 Germantown Avenue, Philadelphia Pa.

On March 10, 2020 at approximately 10:55 a.m., this surveyor arrived on the 7900 block of Germantown Avenue to find a duplex home with no address, and no identifying sign on the building as a business. This surveyor knocked on the door and rang the bell. There was no response. The building appeared empty. There was no evidence that this was the correct address, or that the agency actually resided in this building.

On March 10, 2020 at approximately 11:05 a.m., this surveyor walked to an open business to inquire about how addresses were assigned on the block. The proprietor of the store directed this surveyor to the back of the unlabeled building. Upon reaching the back of the building, the name of the agency, and address was stenciled on the glass door. Peering into the picture window of the office, this surveyor observed a bare desk, and a file cabinet, sitting in the middle of the floor. Upon further observation, a laptop and telephone were on the floor, plugged into the wall. No one was in the office. There was a door next to the office that led to an upstairs apartment.

On March 10, 2020 at approximately 11:07 a.m., by chance the the agency owner may live above the office, this surveyor knocked and spoke to a person identified as a neighbor. The neighbor confirmed that the agency had, "moved out very fast." The neighbor had no forwarding address, and did not know where the agency went.

On March 10, 2020, at approximately 11: 50 a.m., this surveyor arrived at 1542 Haines Street, Philadelphia Pa., a three, (3), story house. The first story of the building appeared abandoned. The side of the building was sealed and appeared abandoned. This surveyor knocked and rang the bell. No one answered. A car drove up and parked in front of the house. In an interview with the driver it was revealed that he live in the next house. He revealed that, "no one lived in the house, and that it was being renovated."
On March 10, 2020, at approximately 12:30 p.m., this surveyor arrived at 6607 Chew Avenue, Philadelphia Pa., (A store front where the previous agency resided and a previous survey was conducted). In an interview with the new occupants of the office it was revealed that, "The previous agency had left, and the office was currently a tax and insurance agency."

On March 10, 2020, between 10:55 and 12:30 p.m., a neighbor at the official address confirmed that the agency had, "moved out," and that the agency left no forwarding address. Previous addresses submitted to the Department revealed no current agency activity. There was no notification to the Department that the agency had moved services to another location.

(d) The home care agency or home care registry shall maintain documentation on file at the agency or registry of compliance with the requirements of this section which shall be available for Department inspection.

Observations:


Based on a Department file review, on-site visit to the agency, interview with a dweller at the official address, and review of the Department's regulations for Home Care Agencies and Home Care Registries, it was determined that the agency failed to make any agency files available for department inspection.

Findings include:

On March 9, 2020, a review of an undated change of address letter to the Department revealed that, "On 9/30/18 the agency would be moving from 6607 Chew Avenue, Philadelphia Pa., to 1542 Haines Street Philadelphia Pa."

The agency or registry shall maintain copies of the criminal history report for the agency or registry owners, which shall be available for Department inspection."

This surveyor was not able to access the Personnel or Client files for Department inspection.
