Based on the findings of an onsite state relicensure survey conducted October 22, 2020, La La's Home Care LLC was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an onsite state relicensure survey conducted October 22, 2020, La La's Home Care LLC was found to be in compliance with the requirements of 28 Pa. Code, Health Facilities, Part IV, Chapter 611, Subpart H. Home Care Agencies and Home Care Registries.

611.51(a) LICENSURE
Hiring or Rostering Prerequisites

Name - Component - 00
Prior to hiring or rostering a direct care worker, the home care agency or home care registry shall: (1) Conduct a face-to-face interview with the individual. (2) Obtain not less than two satisfactory references for the individual. A satisfactory reference is a positive, verifiable reference, either verbal or written, from a former employer or other person not related to the individual that affirms the ability of the individual to provide home care services. (3) Require the individual to submit a criminal history report, in accordance with the requirements of 611.52 (relating to criminal background checks), and a ChildLine verification, if applicable, in accordance with the requirements of 611.53 (relating to child abuse clearance).

Observations:


Based on review of agency policy, consumer record reviews (CF), and staff (EMP) interview, it was determined that the agency failed to ensure two satisfactory references were obtain for five (5) of ten (10) PFs reviewed. (PF1, 2, 5, 7, & 10)


Findings included:


PF review on October 22, 20202 between approximately 10:45am and 11:45am revealed:


PF1, date of hire (DOH) 7/3/202, start of service (SOS) 7/4/2020. PF contained documentation of one satisfactory reference verification dated 7/3/2020. No documentation of second reference verification.

PF2, DOH 7/9/2020,SOS 7/14/2020. PF contained no documentation of satisfactory reference verification. Agency staff unable to locate document.

PF5, DOH 7/21/2020, SOS 7/22/2020, PF contained evidence of one satisfactory reference verification which was not dated. No documentation of second reference verification.

PF7, DOH 8/3/2020. SOS 8/4/2020, PF contained no documentation of satisfactory reference verification.

PF10, DOH 9/1/2020. SOS 9/2/2020, PF contained no documentation of satisfactory reference verification.


Findings confirmed during exit interview with owner and director on October 22, 2020 at approximately 2:00 pm.







Plan of Correction:

Upon review the the Agency will revise Policy 4.10 "Recruiting, Hiring & Rostering" Agency Manager and Supervisor shall form the Screening Committee.Upon a successful hire, the Office clerk shall ensure that the successful candidate has properly filled out all necessary paperwork including two satisfactory reference checks. The office clerk will pass new hire information to Agency Supervisor whom will verify all necessary information including reference checks are properly verified before employee can enter a clients home. The Supervisor will perform monthly new hire audits to double check all new hire folders to ensure all folders are up to date and two verfied reference checks are filled out on the face to face interview.
