Based on a Relicensure Survey completed on October 24, 2023, at Lebanon Valley Birth Center, it was determined there were no deficiencies identified under the requirements of the PA Code, Title 28, Health and Safety, Subsection 501.81 for an existing birth center health care occupancy.
This is a one-story, Type V (000), unprotected wood frame structure, without a basement, which is not sprinklered.
