Based on an Occupancy Survey completed on February 27, 2024, it was determined there were no deficiencies identified as related to the requirements of the Life Safety Code for an existing acute and ambulatory health care occupancy.

This Phase 4 Occupancy Survey was conducted for alterations to replace the rad/fluoro class 1 imaging equipment and to make changes to locker rooms, changing areas, and temporary soiled and clean linen rooms in the imaging suite on the ground floor. Refer to drawing numbers H-23-0541 (original), H-23-0906 (revision), and H-23-1125 (sprinkler).

This Phase 4 Occupancy Survey included the soiled and clean linen rooms and the men's gowned waiting room.

This is an eleven-story, Type II (000), unprotected, non-combustible building, that is fully sprinklered.
