Initial Comments:
Based on the findings of an on-site, unannounced, follow-up survey completed 5/2/24 Connections Life Care had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a state re-licensure and Medicare recertification survey completed 3/8/24.
