Initial Comments:Based on the findings of an offsite follow-up survey conducted December 17, 2024, Concordia Hospice of Bethlen had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of onsite unannounced Medicare Recertification and State Re-licensure survey completed September 13, 2024.
