Initial Comments:
An offsite follow-up survey conducted on April 8, 2024, found that Ascend Hospice had corrected the following deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a state re-licensure survey completed on February 1, 2024.
