Initial Comments:
Based on the findings of an offsite follow-up survey conducted June 1, 2022, Clarion Forest VNA Hospice had corrected the deficiencies cited under 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiencies were cited as a result of a state license and Medicare recertification survey completed April 21, 2022.
