Initial Comments:An offsite follow-up survey conducted on May 21, 2024 found that Crozer Keystone Hospice had corrected the deficiency cited under the requirements of 42 CFR, Part 418, Subparts A, C, and D, Conditions of Participation: Hospice Care. The deficiency was cited as a result of a state re-licensure and Medicare re-certification Survey completed on March 21, 2024.
