Initial Comments:


Based on the findings of an onsite unannounced State relicensure survey completed 4/28/2022, Allison's Careing Koalas Inc. was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an onsite unannounced State relicensure survey completed 4/28/2022, Allison's Caring Koalas Inc. was found not to be in compliance with the following requirement of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries.

Proof of Residency

The home care agency or home care registry may request an individual required to submit or obtain a criminal history record to furnish proof of residency through submission of any one of the following documents:
(1) Motor vehicle records, such as a valid driver ' s license or a State-issued identification.
(2) Housing records, such as mortgage records or rent receipts.
(3) Public utility records and receipts, such as electric bills.
(4) Local tax records.
(5) A completed and signed, Federal, State or local income tax return with the applicant ' s name and address preprinted on it.
(6) Employment records, including records of unemployment compensation

Observations:


Based on a review of the agency policy, personnel files (PF) and staff (EMP) interview it was determined the agency failed to show proof of residency in this Commonwealth for the 2 years preceding the date of hire (DOH) for one (1) of five (5) PFs reviewed (PF3).

Findings included:

A review of the agency policy on 4/27/2022 at 11:10 AM revealed: "...Consumer Service Agreement and Policy...D. HIRING AND COMPETENCY REQUIREMENTS FOR DIRECT CARE WORKERS The direct care worker(s) who will be providing services has met the hiring and competency requirements in accordance with Pennsylvania's homecare licensing regulations. The direct care worker's criminal background checks have been completed and cleared..."

A review of PF3 on 4/27/2022 at approximately 12:00 PM, date of hire (DOH) 8/2/2018 revealed: There was a Pennsylvania driver's license with an issue date of 4/4/2017. There was no additional documentation in PF to show agency verified proof of residency in this Commonwealth for two (2) years immediately preceding the date of hire.

An interview was conducted with the president and care coordinator on 4/27/2022 at approximately 2:30 PM that confirmed the above findings.











Plan of Correction:

It is company policy to require proof of residence 2 years before hire date. We will add a residence section to the hiring procedure employee packet. This section will include conformation that the proof of Pennsylvania residency date is more that 2 years before date of hire. Office staff training will be implemented to introduce this new section and review compliance with Pennsylvania code.
