Plan of Correction:

CRI will update its "Initial Intake Summary" form to include that prior to the commencement of services, the identity of the direct care worker (DCW) who will provide services was reviewed with the consumer.

CRI provides schedules prior to service delivery to allow for consumers to be involved in the service planning process and communicate any issues and/or concerns which they may have.

The Regional Manager of the Lewistown office will meet with staff to educate them on this update to CRI's Initial Intake Summary form prior to 5/30/2025 and be responsible to ensure the update is reviewed with consumers at time of intake.

Over the next 30 days, CRI's Quality Manager will audit files to ensure the update has been made to the Initial Intake Summary form and will monitor for consumer signatures - verifying that this information has been reviewed with them at time of intake. After that, the Regional Manager will be responsible to monitor this POC update to ensure its continued implementation.
