The purpose of this action was to discontinue participation under 42 CFR, Part 483, Subpart G Regulations for Psychiatric Residential Treatment Facilities for residents under the age of 21; effective June 14, 2024. This facility is voluntarily closing due to change in program focus for this agency. The total number of beds closed under this provider agreement is 22 PRTF beds
