Initial Comments:
The purpose of this communication is to ensure that the McKean County Next Generation 9 911 prompted address change (facility and corporate), direct or indirect ownership changes/updates along with all applicable d/b/a(s) are noted with backup, reflected and contained in the following (but not limited to) documents (attestations and CMS-1513s). After months of communications, delays in responding, multiple emails, individual and group telephone calls along with multiple revision reviews of attestations and CMS-1513s, the Division completed the following: (a) facility address change to: 86 Beacon Light Way, Bradford, PA 16701 (b) corporate address change to: 47 Beacon Light Way, Bradford, PA 16701 (the Division directly contacted McKean County Next Generation 9 to obtain additional information for clarification purposes and in turn gave that information to the facility representatives for their records). (c) Accepted the CMS-1513 dated 10-15-24 (received 10-16-24) denoting the above changes and direct and indirect interest ownership information of: Children's Center for Treatment and Education (d/b/a Beacon Light Behavioral Health Systems) and Children's Home of Bradford PA (d/b/a Journey Health System) both addresses 47 Beacon Light Way, Bradford, PA 16701; EIN(s) of 25-1711330 and 25-0965291 respectively. All changes were made (with a completion date of 10-25-24) in both the Commonwealth of Pennsylvania and Federal System(s); backup from the McKean County Next Generation 9 and The PA Department of State were utilized.
The Children's Center for Treatment and Education STAR (d/b/a Beacon Light Behavioral Health Systems and d/b/a Journey Health System) remained in compliance with the Requirements of 42 CFR Part 483, Subpart G Regulations for Psychiatric Residential Treatment Facilities.
