Based on a Relicensure Survey completed on August 17, 2023, at Indiana Ambulatory Surgical Associates, it was determined there were no deficiencies identified under the requirements of the Life Safety Code for an existing ambulatory health care occupancy.
This is a three-story, Type II (000), unprotected, non-combustible building that is fully sprinklered. The ASC is located on the first floor.
