Based on a revisit to a Recertification/Relicensure Survey completed on October 11, 2023, it was determined that Ophthalmology Surgical Center, Inc. was in substantial compliance with the requirements of the Life Safety Code for an existing ambulatory health care occupancy. Compliance with the National Fire Protection Association's Life Safety Code is required by 42 CFR 416.44(b).


This is a three-story, Type III (211), protected ordinary structure, without a basement, which is not sprinklered. The ASC is located on the 2nd floor.
