Initial Comments:
An off site follow up survey conducted on 10/26/2021, found that Community Home Nursing and Therapy Services, Inc. , had corrected the defiencies cited under 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The defiencies were cited as a result of a home health state re-licensure survey completed 9/13/2021.
