Based on a Relicensure Survey conducted May 4, 2021, it was determined that Allied Services Institute of Rehabilitation was not in compliance with the requirements of the Life Safety Code for an existing health care occupancy.

This is a three story, Type II (000), unprotected, noncombustible structure, with a partial basement, which is fully sprinklered.

Sprinklered stories must be sprinklered throughout by an approved, supervised automatic system in accordance with section 9.7. (See 19.3.5)
Give a brief description, in REMARKS, of the construction, the number of stories, including basements, floors on which patients are located, location of smoke or fire barriers and dates of approval. Complete sketch or attach small floor plan of the building as appropriate.


Observations:

Based on observation and interview, it was determined the facility failed to maintain building construction requirements, in six of six smoke compartments.

Findings include:

1. Observation of building construction on May 4, 2021, at 9:15 a.m., revealed the facility exceeded the maximum allowable story height for this type of construction.

Exit interview with facility representative #1 on May 4, 2021, at 9:16 a.m., confirmed the facility exceeded the maximum allowable story height for this type of construction.





Plan of Correction:

Tag K 0161 POC:
The facility submitted an FSES in accordance with the 2012 Life Safety Code to PA DOH on May 31, 2017. Approval was received on the FSES from the Center for Medicare and Medicaid Services (CMS) on August 4, 2017 to ensure ongoing compliance with deficiency K 0161.

Facility requests an update to the current FSES.

Not less than two exits, remote from each other, and accessible from every part of every story are provided for each story. Each smoke compartment shall likewise be provided with two distinct egress paths to exits that do not require the entry into the same adjacent smoke compartment.

Observations:

Based on observation and interview, it was determined the facility failed to provide two acceptable exits from the basement level, affecting one of six smoke compartments.

Findings include:

1. Observation on May 4, 2021, at 9:17 a.m., revealed the basement level lacked a second acceptable exit.

Exit interview with facility representative #1 on May 4, 2021, at 9:18 a.m., confirmed the basement level lacked a required second exit.





Plan of Correction:
