INITIAL COMMENTS
 
This report is a result of an on-site licensure renewal inspection conducted on July 10 through 13, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on August 8, 2006.
 
Plan of Correction

705.24 (3)  LICENSURE Bathrooms.

705.24. Bathrooms. The nonresidential facility shall: (3) Have hot and cold water under pressure. Hot water temperature may not exceed 120F.
Observations
The temperature of the hot water was 140 degrees Fahrenheit.
 
Plan of Correction
WVADS, Inc. will adjust the temperature on the water heater to insure for temperature not to exceed 120 degrees Fahrenheit. The Fiscal Director will be assigned task of insuring compliance.


709.91(b)(6)  LICENSURE Intake and admission

709.91. Intake and admission. (b) Intake procedures shall include documentation of: (6) Psychosocial evaluation.
Observations
Coping/defense mechanisms were missing or incomplete in client records # 2, 3, 4, 5, 6, 8, 9 and 10.
 
Plan of Correction
WVADS, Inc. will conduct specific discussion at next clinical staff meeting on area of coping and defense mechanisms. The Deputy Director/ Treatment Supervisor will insure compliance.

709.92(a)(3)  LICENSURE Treatment and rehabilitation services

709.92. Treatment and rehabilitation services. (a) An individual treatment and rehabilitation plan shall be developed with a client. This plan shall include, but not be limited to, written documentation of: (3) Proposed type of support service.
Observations
Support services were missing on treatment plans in client records # 1, 2, 7 and 8.
 
Plan of Correction
WVADS, Inc. will conduct specific discussion at next clinical staff meeting on area of support services. The Deputy Director/Treatment Supervisor will insure for compliance.
