This report is the result of a complaint inspection conducted on December 28, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the complaint investigation, Clem-Mar House, Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The allegations were not substantiated, however, the following deficiency was identified during the inspection. |

Observations Based on a physical plant inspection, the facility failed to prohibit the use of heaters not permanently mounted or installed.
The findings include:
A physical plant inspection conducted on December 28, 2011 between the hours of 09:30 AM and 10:30 AM revealed that a portable heater was located in the clinical supervisor's office on the first floor of the facility. The supplemental heating system was not permanently mounted or installed.
The clinical director confirmed the presence of the supplemental heating system on December 28, 2011.

Plan of Correction On the date of the inspection, after being notified by the State Reviewer of the presence of the heater, the facility's maintenance tech removed the heater from the premises. After the reviewer left, the maintenance tech then reviewed the full facility to ensure each office was in compliance. To prevent recurrence, the facility will be inspected on a weekly basis by the Clinical Director to ensure full compliance with the regulation. |
